How to search and access intelligently on content based image database become an important focus in the field of multimedia research. In this study, a preliminary system of grid-distributed, contextual and semantic based, intelligent information access framework for medical images database was designed to integrate with radiology reports and clinical department information. A prototyping was established with a model of intelligent diagnostic and therapeutic decision support for brain degenerative diseases among radiologist and neurologist. Dementia was the first disease selected for implementation. In the future, the system will extend its spectrum of diseases and more clinical specialties.} }
